Sports Medicine

Crandall ISD values the health and safety of all student-athletes. We've provided access to important health and safety information for students, parents and coaches.

Information on Annual Physicals: 

All student-athletes 7th-12th grade will need an annual physical on file prior to athletic participation. The physical will need to be completed AFTER May 1st to be current for the upcoming fall season/school year.  For example: a physical dated May 1st, 2022 (or later) will be current for the Fall 2022 season/school year.  A physical dated before May 1st, 2022 will not be accepted for the Fall 2022. 

Once your physical exam is complete, please turn it in to the Athletic Training Staff on your campus, or your coach, on the first day of your season or first day of school (which ever comes first). 

Information on Rank One Online Forms: 

All parents/guardians of 7th-12th grade student-athletes will need to complete the Rank One Online Medical forms annually prior to athletic participation.  

Incoming 7th graders information will not be available in the Rank One system until August 1st each year.  At that point, they will be able to complete their forms. 

New enrollments will be available in Rank One ~48 hours after receiving their student ID.
