New Hire Checklist

Personnel File Requirements

Employee must have on file, or documented evidence of being requested, the items listed before they are eligible to receive their first paycheck:

• Application
• Official Transcripts if Professional (Bachelor and Master Degrees)
• Copy of High School Diploma or GED if auxilary employee
• Copy of Teaching Certificate or Paraprofessional Certificate if required
• Copy of ExCet scores for beginning teachers that have not received their certificate
• Copy of Social Security Card
• Copy of Driver's license (to establish identity)
• I-9 Form
• W-4 Form
• Criminal History Forms 1 & 2
• Public Information Access Notification Form
• Statement Concerning Not Covered by Social Security
• Vehicle Permit Information
• 403 (b) Form
• Request for Service Record from previous school employer
• Employee Agreement for Acceptable Use
• Employee Children Attending CISD
• Job Description provided by central office
• Signed Contract or Letter of Reasonable Assurance provided by central office
• Visit with Employee Benefits Department


Please Supply all items to complete your personnel file.

YOUR FIRST PAYCHECK WILL NOT BE ISSUED IF ANY OF THE ABOVE ITEMS ARE MISSING.
